Fellowship Application — Fill-up Guide

Step-by-step instructions to help you apply and complete the fellowship application form easily.

How to Apply
  1. Complete all 5 steps in order — or jump using the step bar on the form.
  2. Click Next Step to save and continue.
  3. Use Save & Resume to pause and return later.
  4. Keep your Application ID safe for resuming.

Step-by-Step Guide
Step 1: Fellowship Selection

Choose the fellowship area you want to apply for.

  • Select one fellowship specialty — this is required.
  • Read the short description or open Details for full information.

Step 2: Personal & Family

Enter personal details, contact information, addresses, and optional family information.

  • Use your full legal name as on academic certificates.
  • Save your email here if you want to resume with ID + email later.
  • Upload a clear passport-size photo (JPG or PNG).
  • Family fields are optional — fill only what applies to you.
Required for submit: Full name · Gender · Date of birth · Nationality · Mobile · Email · Communication address
Step 3: Education & Career

Add academic qualifications, additional training, and professional experience.

  • List degrees from MBBS upward using the + button.
  • Include workshops and short courses in the training section.
  • Enter current designation, medical registration, and employment history.
Required for submit: Current designation · Medical registration number · Years of experience · At least one qualification for final submission
Step 4: References & Statement

Provide references, research activities, and your statement of purpose.

  • Add at least one senior consultant or supervisor as a reference.
  • Share publications, awards, and conference activity if applicable.
  • Minimum 100 characters required for statement of purpose on submit.
Required for submit: At least 1 reference · Statement of purpose (100+ characters on submit)
Step 5: Documents & Submit

Upload supporting documents, declare accuracy, and submit your application.

  • Each file must be under 10 MB (PDF, DOC, DOCX, JPG, or PNG).
  • Photograph, CV, National ID, and medical registration are important.
  • Tick the declaration and type your full name as electronic signature.
